Dr. Trevor Baker
Trevor Baker PT, DPT, GCS is a Board-Certified Clinical Specialist in
Geriatric Physical Therapy, bringing advanced expertise in the care of
older adults. He earned his Bachelor of Science in Exercise Science from
Utah Valley University, where he was a four-year student-athlete in cross
country and track and field, and his Doctor of Physical Therapy degree
from The University of Utah.
Dr. Baker has extensive experience working with older adults across
home, outpatient, and assisted living settings, with a strong focus on fall
prevention, balance training, mobility restoration, and maintaining
independence.
He is passionate about helping patients improve their quality of life and
works closely with families to develop individualized care plans focused
on safety, confidence, and long-term well-being—all delivered in the
comfort of the patient’s home, assisted living, independently living, and/or
memory care facility.
About The Clinic
Baker Mobile Therapy was founded on a simple belief: healthcare should prioritize people—not just processes or payment models. After years in traditional and corporate healthcare settings, it became clear that care was often driven more by reimbursement structures than by what patients truly needed to improve their quality of life.
In 2020, Dr. Baker transitioned to private practice to create a model centered on high-quality, individualized care—one that allows patients to receive the right care, at the right time, in the environment where they live and function every day. By working collaboratively with physicians and navigating insurance appropriately, the focus remains where it should be: on the patient.
Since its founding, Baker Mobile Therapy has helped older adults improve mobility, reduce fall risk, and maintain independence through mobile outpatient physical therapy delivered directly in the home, as well as in assisted living, independent living, and memory care communities.
A cornerstone of the practice is a personalized fall prevention approach, along with targeted treatment of musculoskeletal, neurologic, and balance-related conditions. As a board-certified clinical specialist in geriatric physical therapy, Dr. Baker brings advanced expertise to help patients age safely and confidently in place.
This model not only benefits patients and their families—but also supports community partners. Assisted living and memory care communities often see improved resident satisfaction, reduced fall-related incidents, and greater long-term stability for their residents.
